He may get more than 5 mil. From the original story:
"The lucky doorman actually won the prize May 2, when he bought a $10Set for Life instant ticket, which pays $5,000 a week for life and guarantees a minimum of $5 million. Winners get $65,000 every three months, or $41,926 after taxes." (emphasis, mine)
I agree on the anonymity thing. hmm... i thought the set for life has a MAXIMUM payout not minimum payout. I will have to look at the one here for TX.

Here's an example that's less than a year old.
"As the winner of the Lottery's Set For Life instant game, Gerard Wilson is guaranteed to receive a "Minimum Payout" of $5 Million dollars, to be paid in Quarterly installments for AS LONG AS HE LIVES."

So, it looks like our beloved Doorman will be paid for quite some time. At least until he actually dies.

I dunno, but on one of her websites, it says she frequents New York due to her Modeling career. Well, he is a doorman at apparently quite an upscale Apt./Condo complex. There may be a tie-in somewhere!! But of course, I truly don't know. Your guess is probably better than mine!
