Notice that the ban IS FROM collecting prizes and not from playing; What bothers me is BANED FOR LIFE ???/ sounds like you kill someone so you get life sentence,but baned for life in collecting prizes

That's just stupid. Seems like the lottery just want to be able to save some money. If they win larger sums they will just go to a friend and ask him/her to cash the check for him anyway. There's no real way of stopping gamblers who want to play no matter what.

To me, it's useless.

If I am a 'problem gambler', I may try to convince myself that by signing this form it would 'assure' (me) that I would not gamble (much), but just like some alcoholics going through the motions with Al-Anon, I would find a way around it, like having a friend claim any large prizes for me.

It is well-intended, and may work for some, but I fail to see what good a dollar limit on the payout does: you are either a gambler, or not. This limits how much you can collect if... [ More ]
