Yup. If I had a scanner I'd scan one in. Maybe someone here on LP from IL can.

When my wife saw this she said she wondered what they'd do if you sent a dog in with $2 a playslip in its collar, and then if it won, the news would say, Local dog wins Powerball.

If you think about it, pet dogs do have ID's and they couldn't expect them to say too much at the press conference!
