The middle width is what I use in which the second highest number is subtracted from the largest.

Example

180

Width is zero to eight=8.

The middle width is 1 to 8 which is the difference of 7.

Example

Width 5

Middle width 4.

Question regarding Middle Width: I presume that Lottery Math takes over in a case like 348 . . . Middle Width is 3 - 4 = -1 = 9? If not, please set me straight.
