Whoa... California.... tsk tsk tsk , your volunteer info sections need severe updating...no section for indigenous people ..only male or female for the gender ?...not good

Sounds like your intent is Tax evasion which often entails taxpayers deliberately misrepresenting the true state of their affairs to the tax authorities to reduce their tax liability and includes dishonest tax reporting, such as declaring less income, profits or gains than the amounts actually earned, or overstating deductions!

Your SS# on the form is for tax purposes. Doctors use it for patient identification. If you turn it in without it, eventually they will find you as you put it, when you file your taxes next year. But why do you want to risk having your winnings held up, an IRS audit, or them questioning why you left it off? In other words, why open up a can of worms if you don't have to. Hello, I know in CA, undocumented people can still claim prizes. The CA claim form has a check box that says I don't have an SS# . They simple are subject to 28% or 30% withholding Vs 25% for SS# holders.

My question is I do have an SS#, but I don't want to write on the claim form with all this ID theft going on. Even at the Drs office I never put my SS# on their forms.

My address is the same for the last 23 years, this should suffice right? I'm sure they can find me and lookup t... [ More ]
